WesleyLife is seeking a dedicated Physical Therapist to provide in-home, one-on-one therapy focused on mobility, strength, and independence. You’ll work directly with clients in their homes, assessing needs and creating personalized treatment plans with evidence-based techniques.

You will collaborate with a multidisciplinary team, educate clients and caregivers, and monitor progress to optimize outcomes. This role emphasizes compassionate, family-centered care delivered in patients' home

Qualifications

• Valid Iowa driver's license.
• Current auto insurance.
• Licensed as a Physical Therapist in Iowa.
• Current Basic Life Support certification.
• Doctor of Physical Therapy degree from a PT education program.

Responsibilities

• Deliver one-on-one home-based physical therapy to improve mobility and function.
• Assess clients in their homes and develop individualized treatment plans.
• Provide tailored exercises, manual techniques, gait training, and evidence-based interventions.
• Educate clients and caregivers on safety, pain management, and home modifications for independence.
• Collaborate with interdisciplinary team to coordinate care and align with client goals.
• Monitor progress and adjust plans as needs evolve.
• Communicate progress and adjust plans based on client needs and team input.
