Job summary

CenterWell Home Health, a Humana company, is hiring a Home Health Physical Therapist to deliver in-home care that improves function and independence. You will collaborate with a clinical team to create personalized treatment plans and document progress meticulously.

Responsibilities include planning and administering PT services at patients' homes, guiding PTAs and aides, and reporting outcomes to physicians and the care team. A patient-centered approach is essential.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide goals.
• Develop and implement a rehabilitation program aligned with the physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Embed patient-centered care to make an impact.
