Job summary

CenterWell Home Health, part of Humana, seeks a Physical Therapist to provide in-home therapy, helping patients regain strength, mobility and independence. You will work under the Clinical Manager to plan and administer treatments tailored to each patient’s goals and safety needs.

Responsibilities include assessing function, documenting progress, supervising PTAs and aides, coordinating with the care team, and communicating with patients and families to ensure optimal recovery.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes.
• Assess patient strengths to support physician evaluation.
• Develop and implement rehab program aligned with Plan of Treatment.
• Provide PTAs and Home Health Aides with clear care instructions and oversee documentation.
• Document patient observations, interventions, and evaluations.
• Report progress to the physician, Clinical Manager, care team, patient, and family.
