Job summary

CenterWell Home Health, a Humana company, is seeking a Home Health Physical Therapist to deliver in-home care aimed at improving strength, mobility and independence. You will assess patients, plan individualized PT programs, and supervise PTAs and aides as part of a multidisciplinary team.

Responsibilities include documenting progress, coordinating with physicians and care teams, and reporting patient outcomes.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient and family, and submit all required evaluations and discharge summaries.
