Job summary

CenterWell Home Health, part of Humana, is seeking a Physical Therapist to deliver in-home therapy, helping patients regain strength, mobility and independence. You will work with a clinical team to plan and execute care in patients' homes.

Responsibilities include planning therapy, documenting progress, supervising PTAs and Home Health Aides, and coordinating with physicians. This is a full-time position with TB screening and driver-safety requirements, and a comprehensive benefits package.

Qualifications

• Must hold a current, unrestricted Physical Therapy license.
• CPR certification is required.
• Experience collaborating with healthcare professionals as part of a team.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide clear instructions to PTAs and Home Health Aides and oversee documentation.
• Document patient observations, interventions, and progress promptly.
• Report progress to physician and care team, and submit evaluations and discharge summaries.
