Job summary

CenterWell Home Health, a Humana Inc. division, is seeking a Home Health Physical Therapist to visit patients in their homes and help them regain strength, mobility and independence through personalized care.

You will assess patients, plan and administer prescribed therapy, guide rehabilitation, supervise PTAs and aides, document progress, and communicate with physicians, the care team, patients and families.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes.
• Assess and document patients' physical strengths to guide goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide PTAs and Home Health Aides with written care instructions and supervision.
• Document observations, interventions and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient and family; submit evaluations and discharge summaries.
