CenterWell Home Health in the United States is seeking a licensed Physical Therapist to deliver in-home care, helping patients regain strength, mobility and independence. You will plan and administer prescribed PT treatment onsite in patients' homes, document progress, and collaborate with the care team to achieve optimal outcomes.

This role requires an unrestricted PT license and CPR certification, with a typical 40-hour workweek and a pay range of $93,000 to $128,000 annually, plus

Formación

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsabilidades

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with the physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
