Job summary

AccentCare is seeking a Home Health Physical Therapist to deliver skilled, compassionate care in patients’ homes. You will assess mobility, create personalized plans, and guide exercises to restore strength, balance, and independence.

You’ll educate families, monitor progress, and adapt plans in collaboration with the care team to ensure safe, timely interventions and meaningful improvements in daily living.

Qualifications

• Master’s degree in physical therapy approved by a CAPTE-accredited physical therapist education program.
• Current physical therapist license within the state of operation.
• Valid CPR certification.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Conduct thorough patient evaluations: Assess mobility, functional limitations, safety risks, and therapy needs to establish a clear roadmap for successful in‑home rehabilitation.
• Create personalized treatment plans: Design evidence based, goal driven therapy plans for each patient’s condition.
• Deliver skilled, hands‑on care: Provide one‑on‑one interventions to improve mobility and independence.
• Educate and empower patients and families: Teach home exercise programs and safety techniques to encourage progress between visits.
• Monitor progress and communicate changes: Track results and coordinate with physicians and the care team.
