Job summary

Select Home Health in Bloomington, IN is seeking a compassionate Physical Therapist to provide in-home skilled therapy, helping patients regain independence and improve function under physician plans of care.

You will evaluate and update individualized care plans, deliver treatments to relieve pain and restore function, educate patients and caregivers, supervise PT assistants every 30 days, and collaborate with an interdisciplinary team to ensure high-quality care.

Qualifications

• Graduate of an accredited physical therapy program (APTA or AMA approved).
• Licensed as a Physical Therapist in the state of practice.
• Minimum of 1 year of experience as a Physical Therapist.

Responsibilities

• Provide skilled physical therapy in patients' homes according to physician's plan of care.
• Evaluate patient function and develop/update individualized care plans.
• Deliver treatments to relieve pain, restore function, and maximize performance.
• Monitor progress, document outcomes, and communicate changes to the physician and care team.
• Educate patients, caregivers, and staff on therapy goals and devices.
• Supervise Physical Therapy Assistant at least every 30 days.
• Collaborate with interdisciplinary team for coordinated care.
