Job summary

Select Home Health & Hospice in central Indiana seeks a compassionate Physical Therapist to provide skilled therapy in patients’ homes, helping them regain independence and meet rehabilitation goals. You will collaborate with patients, caregivers, and the care team to develop and implement individualized plans.

The role requires graduation from an accredited PT program, active state licensure, and at least one year of PT experience.

Qualifications

• Graduate of an accredited physical therapy program (APTA or AMA approved).
• Licensed as a Physical Therapist in the state of practice.
• Minimum of 1 year of experience as a Physical Therapist.

Responsibilities

• Provide skilled physical therapy in patients’ homes according to the physician’s plan of care.
• Evaluate and update individualized care plans with patient function goals.
• Deliver treatments to relieve pain, restore function, and improve maximum performance.
• Monitor progress, document outcomes, and communicate with physicians and care team.
• Educate patients, caregivers, and staff on therapy goals and use of equipment.
• Reassess needs regularly and revise plans as needed.
• Collaborate with interdisciplinary team for coordinated, high-quality care.
• Participate in quality improvement initiatives and ongoing education.
• Provide supervision of the Physical Therapy Assistant at least every 30 days.
