Qualifications

• Graduate of an accredited physical therapy program (APTA or AMA approved) is required.

• Valid driver’s license, auto insurance, and reliable transportation are needed.

Responsibilities

• Provide skilled physical therapy in patients’ homes according to the physician’s plan of care.
• Deliver treatments to relieve pain and restore function.
• Monitor patient progress, documenting outcomes and communicating changes.
• Educate patients and caregivers on therapy techniques and adaptive devices.
• Collaborate with interdisciplinary team members to ensure coordinated care.
