Job summary

PARS Therapy in Dayton, TX is seeking a compassionate Physical Therapist to join our home health team. You will evaluate patients at home, design personalized treatment plans, and help clients regain mobility and reduce pain.

The ideal candidate holds an active Texas PT license, has home health experience, and embraces evidence-based care. You will educate families, document progress, and collaborate with a multidisciplinary team to support patient independence.

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage for Texas PT license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with interdisciplinary team members including physicians, nurses, and therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
