Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Home Care Physical Therapist for Western Nassau County to provide in-home services. Flexible scheduling with 1099 and W-2 options to fit your needs.

We train new graduates and welcome all levels of experience. Travel to patients’ homes within the designated area is required, with a focus on quality patient care and independence.

Qualifications

• Physical Therapist program graduate and eligible for NY licensure.
• Licensed Physical Therapist in NY or eligible.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in home care and create individualized plans.
• Develop plan of care and goals in consultation with physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function.
• Instruct family members, PTAs, Home Health Aides and caregivers on therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate with patients, families, and interdisciplinary team, especially discharge planning.
• Deliver care in compliance with agency policies and professional standards.
