Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Physical Therapist for in-home, part-time work in New York. You will assess patients, develop personalized plans, and collaborate with physicians and families to promote independence.

We offer mentorship, strong benefits, and flexible scheduling across the New York region. Travel to client homes is required, with a focus on quality patient care and documentation accuracy.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in NY or eligible.
• Home health experience preferred; mentorship provided.
• Ability to travel to patients' homes within the territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate home-based PT needs and develop treatment plans.
• Plan care with physician, patient and family.
• Implement customized therapy to restore function.
• Instruct family, PTAs, HHAs, and caregivers on therapy techniques.
• Provide timely documentation for each visit.
• Communicate with patients, families and interdisciplinary team, especially at discharge.
• Deliver care per agency policies and standards.
