Job summary

Metro Physical & Aquatic Therapy in Western Suffolk County is seeking a part-time Home Care Physical Therapist to provide in-home care with comprehensive benefits. The role involves evaluating patients, designing individualized treatment plans, and coordinating with physicians and families to promote independence.

Applicants must be a licensed NY Physical Therapist or eligible to become licensed; home health experience is preferred and travel to patients’ homes within the designated geographic

Qualifications

• Licensed NY PT or eligible to obtain license.
• In-home care experience preferred but not required.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
