Job summary

Metro Physical & Aquatic Therapy is seeking a Home Care Physical Therapist to deliver in-home care on Long Island. The position is full-time with a competitive salary and a comprehensive benefits package.

All levels of experience are welcome; mentoring is provided for new graduates. You’ll join a supportive, multidisciplinary team, enjoy flexible scheduling to see patients in your geographic area, and work with modern tools to ensure excellent patient care.

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in New York or eligible
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards

Education

Physical Therapist Program Graduate

Licensed Physical Therapist in New York or eligible
