Metro Physical & Aquatic Therapy is seeking a motivated and passionate part-time Physical Therapist to provide in-home care across New York. All levels of experience are encouraged and will be trained for success within a growing, supportive team.

Qualifications

• Licensed Physical Therapist in NY or eligible is required or eligible for licensure in NY.
• Graduate of a Physical Therapist program.
• Ability to travel to patients' homes within designated geographic area.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.
