Homeland HomeHealth is seeking a compassionate Physical Therapist to provide in-home rehabilitation services. This part-time role offers flexible hours (roughly 20–30 per week) and requires a PA license and reliable transportation to patients' homes.

You will assess patients referred by physicians, develop treatment plans, and work within a multidisciplinary team to promote healing and independence while delivering high-quality care in the comfort of patients' homes.

Qualifications

• Must be licensed as a Physical Therapist in Pennsylvania.
• Minimum two years' experience as a physical therapist.
• Home health experience preferred.
• Ability to work in a multidisciplinary team.

Responsabilités

• Evaluate and provide treatment to HomeHealth patients referred by physician.
• Work as part of an interdisciplinary team to help patients achieve goals.
• Provide treatment in patients' homes.
• Follow organization policies and state regulations.
