Job summary

Four Seasons Certified Home Health Agency in Brooklyn, NY seeks a compassionate Fee-For-Service Physical Therapist to deliver high-quality home health care. You will evaluate patients, plan treatment, and provide therapeutic interventions in the patient’s home environment.

Responsibilities include developing care plans, coordinating with families and clinicians, documenting visits and ensuring compliance with CHHA standards. Prior experience in home health preferred.

Qualifications

• Active New York Physical Therapy license.
• Valid driver license and access to a car.
• USA work authorization.
• Experience in home health or CHHA with OASIS/PDGM knowledge.
• Strong communication, organization and leadership; EMR proficiency.

Responsibilities

• Assess patients, develop and update individualized home health care plans.
• Coordinate care with patients, families and clinicians.
• Document visits, authorizations, and Oasis data; ensure timely billing.
• Provide hands-on physical therapy, exercises and modalities.
• Educate patients and families on home exercises and safety.
• Evaluate home safety and suggest fall-prevention modifications.
• Collaborate with the care team for comprehensive patient care.
