Four Seasons Certified Home Health Agency in Brooklyn, NY is seeking a compassionate Fee-For-Service Physical Therapist to deliver high-quality home health care. You will evaluate patients, plan treatments, and guide exercises in patients’ homes, coordinating with the care team for optimal outcomes.

The role requires NY licensure, a valid driver's license and access to a car, and experience with home health care, OASIS, and PDGM processes.

Qualifications

• Licensed NY PT required.
• Driver license and own car required.
• Experience in home health or CHHA settings valued.
• Knowledge of OASIS, 60-day episodes, PDGM and 30-day Reassessment is highly valued.

Responsibilities

• Collaborate with patients and families to develop individualized care plans.
• Enter visit frequencies and service requests; document for auth and Oasis as needed.
• Assess patients in their home environment and evaluate medical history and function.
• Develop evidence-based treatment plans to improve strength, balance, flexibility, and ROM.
• Provide hands-on therapy and modalities to manage pain and restore function.
• Train patients and families on equipment use and home safety.
• Educate patients on condition, recovery, and home exercise programs.
• Evaluate home safety and recommend modifications to reduce falls.
• Maintain accurate patient records and report progress to physicians and care team.
• Collaborate with nurses and social workers for coordinated care.
