Job summary

@Home Therapy is seeking a licensed Physical Therapist to provide in-home rehabilitation to adult patients in Brooklyn, NY. You will evaluate gait, balance, and mobility in the home and develop individualized treatment plans to restore function and independence.

Qualifications

• Active New York State Physical Therapist license.
• Experience with adult rehabilitation preferred.
• Ability to work independently in home care.
• Strong communication and clinical reasoning skills.

Responsibilities

• Assess home mobility: balance, gait, strength, ROM, functional mobility.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Train in safe transfers, ambulation, stairs as applicable, and home mobility.
• Monitor progress, reassess status, adjust plans as needed, and collaborate with physicians.
