Qualifications

• Active NY Physical Therapy license or eligibility.
• Driver license and access to a car.
• Experience in home health or CHHA settings with OASIS/PDGM knowledge valued.
• Strong communication, organization, and leadership skills; EMR proficiency.

Responsibilities

• Collaborate with patients, families, and healthcare professionals to develop individualized care plans.
• Enter visit frequencies and request services; document notes for authorization and Oasis when needed.
• Evaluate patient abilities, medical history, and home environment.
• Develop evidence-based treatment plans to improve strength, balance, flexibility, and range of motion.
• Provide hands-on therapy and therapeutic exercises; use modalities as needed.
• Train patients and families to use canes, walkers, wheelchairs, and transfer devices.
• Educate patients on condition, recovery, and home exercise programs.
• Assess home safety and recommend modifications to reduce fall risk.
• Maintain accurate patient records and report findings to the physician and care team.
• Coordinate care with nurses, social workers, and other professionals.
