Job summary

Four Seasons Certified Home Health Agency in Brooklyn, NY seeks a compassionate Occupational Therapist to provide high-quality home health services under a fee-for-service model. You will evaluate patients, plan treatments, deliver hands-on therapy, and coordinate care with families and medical teams.

Responsibilities include documentation, care plan development, home safety assessments, and ensuring compliance with CHHA standards. In this role, strong communication and EMR skills are essential.

Qualifications

• Licensed Occupational Therapist in New York State.
• Driver's license and a car are required.
• Authorized to work in the USA.
• Experience in home health or CHHA with OASIS, 60-day episodes, 30-day PDGM and 30-day Reassessment knowledge.
• Strong communication, organization, leadership and EMR proficiency.

Responsibilities

• Coordinate patient care plans with patients, families and health professionals.
• Enter visit frequencies and service requests; document notes for auth and Oasis.
• Evaluate patients' abilities, medical history and functional limitations in the home.
• Develop evidence-based treatment plans and deliver hands-on therapy.
• Educate patients/families, train on equipment, and ensure home safety; report progress.
