Job summary

@Home Therapy is seeking a licensed Physical Therapist to provide in-home rehabilitation services to adult patients in Manhattan.

You will assess mobility, implement personalized plans, and coach patients and caregivers on fall prevention and safe movement in real-world settings.

This field-based role offers flexible scheduling, supportive admin staff, and the opportunity to help patients maintain independence at home.

Qualifications

• Active New York State Physical Therapist license.
• Experience with adult rehabilitation preferred.
• Strong gait, balance, and functional movement training knowledge.
• Ability to work independently in patient homes.
• Strong communication and clinical reasoning skills.

Responsibilities

• Perform home mobility assessments evaluating balance, gait, strength, range of motion, and functional mobility.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Provide real-world movement training for safe transfers, ambulation, and stair negotiation when applicable.
• Monitor progress and adjust treatment plans based on clinical findings in the home setting.
• Document care timely in the EMR and maintain compliance with standards.
