Job summary

@Home Therapy is seeking a Licensed Physical Therapist to provide in-home rehabilitation to adult patients in Manhattan. The role focuses on improving mobility, restoring functional movement, and supporting independence within the patient’s home environment.

You will evaluate balance, gait, strength and functional mobility, develop individualized treatment plans, educate patients and caregivers on fall prevention, and monitor progress while coordinating with physicians and the care team.

Qualifications

• Active NYS Physical Therapist license.
• Experience with adult rehabilitation preferred.
• Strong communication and clinical reasoning skills.
• Ability to work independently in home care setting.
• Knowledge of gait, balance and functional movement training.

Responsibilities

• Evaluate balance, gait, strength, ROM, and functional mobility in the home environment.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Train patients in safe transfers, ambulation, stair negotiation and household mobility.
• Educate patients and caregivers on fall prevention and safe movement strategies with assistive devices.
• Monitor progress, reassess status, and modify plans; collaborate with physicians and care team.
• Complete timely documentation in the EMR and maintain compliance with standards.
