Job summary

@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adult patients in the greater Cherry Hill area. This role focuses on improving mobility, restoring functional movement, and helping patients maintain independence within their home environments.

You’ll assess balance, gait, strength, ROM in the home, develop individualized plans, train in transfers and household mobility, educate patients and caregivers on fall prevention, monitor progress, document in

Qualifications

• Provide in-home physical therapy to adult patients in a community-based setting.
• Develop and implement individualized treatment plans for mobility and independence.
• Educate patients and caregivers on fall prevention and safe movement.

Responsibilities

• Evaluate balance, gait, strength, ROM, and functional mobility in the home.
• Identify barriers to safe mobility and independence in home environments.
• Develop and implement rehab plans to improve mobility, strength, and balance.
• Train patients in safe transfers, ambulation, stair negotiation when applicable.
• Monitor progress, reassess status, and adjust plans with the care team.
• Complete timely documentation in the EMR and maintain regulatory standards.
• Deliver care directly in patients' homes with flexible scheduling.
