Job summary

@Home Therapy seeks a Physical Therapist to provide in-home rehabilitation to adult patients in the Jericho area, focusing on mobility, independence, and function within the home environment.

You will assess balance and gait, design individualized treatment plans, educate patients and caregivers on safety and fall prevention, document in the EMR, and collaborate with the care team while enjoying flexible scheduling and field-based independence.

Qualifications

• Active New York State Physical Therapist license.
• Experience with adult rehabilitation preferred.
• Strong understanding of gait, balance, and functional movement training.
• Ability to work independently in a home care setting.
• Strong communication and clinical reasoning skills.

Responsibilities

• Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
• Identify environmental and physical barriers that may affect safe mobility and independence.
• Develop and implement individualized treatment plans to improve or maintain mobility, strength, balance, and functional independence.
• Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.
• Train patients in safe transfers, ambulation, stair negotiation (when applicable), and household mobility.
• Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.
• Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
• Collaborate with physicians and the interdisciplinary care team to support patient goals.
• Complete timely documentation using the agency's electronic medical record (EMR) system.
• Maintain compliance with clinical, regulatory, and agency documentation standards.
