Job summary

@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation for adults in the greater Cherry Hill area. This role focuses on improving mobility, restoring functional movement, and helping patients maintain independence within their home environments.

You will evaluate, treat, and educate patients in real-world movement, with flexible scheduling and field-based independence.

Qualifications

• Active NJ State Physical Therapist license.
• Experience with adult rehabilitation preferred.
• Strong understanding of gait, balance, and functional movement training.
• Ability to work independently in home care setting.
• Strong communication and clinical reasoning skills.

Responsibilities

• Evaluate balance, gait, strength, range of motion, and functional mobility in the home.
• Identify barriers affecting safe mobility and independence.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and independence.
• Provide rehabilitation for adults recovering from illness, injury, surgery, or managing chronic conditions.
• Train patients in safe transfers, ambulation, stairs (when applicable), and household mobility.
• Educate patients and caregivers on fall prevention and safe movement strategies.
• Monitor patient progress, reassess status, and modify treatment plans as needed.
• Collaborate with physicians and interdisciplinary team to support patient goals.
• Complete timely EMR documentation and maintain compliance with standards.
