@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services for adult patients in Brooklyn, NY. • Active NY state PT license required.
• Experience in adult rehabilitation preferred.
• Strong knowledge of gait, balance, and functional movement training.
• Ability to work independently in a home care setting.
• Strong communication and clinical reasoning skills.

Responsabilidades

• Perform home mobility assessments evaluating balance, gait, strength, ROM and functional mobility.
• Develop and implement individualized therapy plans to improve mobility, strength, balance and independence.
• Provide real-world movement training for safe transfers, ambulation and stair negotiation in home settings.
• Monitor progress, reassess status and adjust treatment plans with the care team.
