@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adults, focusing on mobility, functional movement, and independence within patients' homes. Service area includes South Queens, NY; flexible scheduling and field-based independence support delivering care with minimal clinic constraints.

Qualifications

• Active NY PT license required.
• Experience with adult rehabilitation preferred.
• Strong gait, balance and functional movement training knowledge.
• Ability to work independently in home care setting.
• Strong communication and clinical reasoning skills.

Responsabilités

• Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
• Identify environmental and physical barriers that may affect safe mobility and independence.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.
• Train patients in safe transfers, ambulation, stair negotiation, and household mobility.
• Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.
• Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
• Collaborate with physicians and the interdisciplinary care team to support patient goals.
• Complete timely documentation using the agency's EMR system and maintain compliance with standards.
