Job summary

Providence at Home with Compassus is seeking a Physical Therapist to perform home health evaluations, treatment planning, and discharge planning for clients in the Napa, CA area. You will work with a multidisciplinary team to coordinate care and ensure high-quality patient outcomes while supervising therapy staff.

The role requires a PT license in good standing, CPR/BLS certification at hire, and preference for 1 year of experience with adult physical disabilities.

Qualifications

• Must hold a Bachelor’s Degree in Physical Therapy.
• Current State Licensure required within 30 days of hire.
• CPR/BLS certification required at hire.
• Preferred 1 year of therapy with adult physical disabilities.

Responsibilities

• Performs comprehensive patient assessments to establish/update treatment plans.
• Administers physician-ordered PT programs to restore function and prevent disability.
• Records and evaluates assessments, plans, and progress notes for care coordination.
• Supervises PT assistants and aides to ensure quality and compliance.
• Collaborates with physicians and the multidisciplinary team to coordinate care.
