Well Care Health is seeking a compassionate Home Health Physical Therapist to join our team and provide exceptional patient care in patients’ homes across the Carolinas.

The role requires a PT degree from an accredited university, NC/SC licensure, and CPR certification, with a minimum of 1 year of experience. Clinicians collaborate with a supportive clinical manager and can help families manage health at home.

• Graduation from Physical Therapy program from an accredited university.
• Licensure in NC/SC; CPR certification.
• Minimum 1 year of experience requested.
• Ability to perform home health PT procedures and use related equipment.

Responsabilidades

• Provide home-based physical therapy in patients' homes.
• Monitor and report patient changes; adjust care plans.
• Educate patients/families on health management and self-care.
• Collaborate with care team to ensure high-quality care.
• Document assessments and treatment plans per policy.
• Participate in program development and performance improvement.
