Job summary

Well Care Health is seeking a compassionate Home Health Physical Therapist to join our team and provide care in patients' homes. This role includes assessing, planning, and delivering physical therapy and educating families on health management.

Eligible candidates hold a Physical Therapy degree and NC/SC licensure, with CPR certification. Minimum 1 year of home-health experience is preferred; travel and driving are required. Competitive salary and benefits offered.

Qualifications

• Graduate of Physical Therapy program from an accredited university.
• Licensure in the state where providing therapy (NC/SC); CPR certification.
• Minimum 1 year of experience in home health or related setting.

Responsibilities

• Assess, plan, implement and evaluate patients with appropriate skill for age/diagnosis.
• Deliver care in patients' homes and document in accordance with policies.
• Educate patients and families on health management and self-care techniques.
• Collaborate with the care team to ensure high-quality home health services.
• Maintain schedules and contribute to program development.
