Job summary

Scylla Solutions LLC is seeking a licensed Physical Therapist to provide outpatient rehabilitation in home-based settings for older adults, evaluating and treating in the patient’s home or in independent living facilities. You will design individualized therapy programs, educate patients and caregivers, and coordinate with other rehab team members to promote independence and safety.

Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master’s in a related field with a PT degree.
• Bachelor's degree in Physical Therapy with 5 years of progressive PT experience.
• Qualifying PT degree earned outside the U.S. must be evaluated for US equivalence.
• Must have a valid driver's license or reliable transportation to patients' residences.
• License eligibility may be acceptable in certain cases.

Responsibilities

• Supervise Physical Therapy Assistants and temporary licensees per state rules.
• Adhere to patient treatment schedules and complete timely documentation.
• Attend patient care conferences and contribute to team discussions.
• Refer to other disciplines to address comprehensive patient needs.
• Consult with rehabilitation team to select the most appropriate therapy regimen.
• Adapt plan of care to individual patient needs.
• Promote safety and eliminate hazards in home environments.
• Order supplies and maintain equipment as necessary.
• Educate patients and caregivers to promote independence and skills.
• Perform home and community mobility assessments.
