Job summary

Fox Rehabilitation seeks licensed Physical Therapists in the Alexandria, VA area to provide one-on-one geriatric care in patients' homes or Senior Living Communities. You will assess, plan, and deliver personalized therapy, documenting in FOX’s EMR and collaborating with a multidisciplinary team.

The role offers autonomy, flexible scheduling, and potential telehealth components to extend access and continuity of care. New graduates welcome through mentorship programs.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system in line with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
