Job summary

FOX Rehabilitation is expanding in Stafford, VA and seeks licensed Physical Therapists to deliver one-on-one care to older adults in homes and senior living communities. You’ll design individualized care plans, perform evaluations, and use evidence-based treatments to improve mobility and independence.

The role supports autonomy and flexible scheduling, with opportunities to integrate telehealth for extended access and continuity of care.

Qualifications

• Licensed Physical Therapist in the state of practice or eligibility to obtain licensure.
• Graduated from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes and senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth or virtual care when appropriate to extend access.
