Job summary

FOX Rehabilitation is growing in Arlington, VA and is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living communities. Clinicians enjoy autonomy, flexible scheduling, and the opportunity to build lasting patient relationships through our Geriatric House Calls model.

The role may include treating in private residences or senior living communities, with optional telehealth, to improve mobility and independence for older adults.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
