Job summary

Fox Rehabilitation seeks licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living communities in the Fair Lakes, VA area. You will evaluate, plan, and implement customized treatment plans while promoting independence and mobility.

Clinicians may incorporate telehealth to extend access and flexibility, with a schedule you can manage. The role emphasizes autonomy, meaningful outcomes, and collaboration within an interdisciplinary team.

Qualifications

• Licensed Physical Therapist in VA or eligible to obtain licensure.
• Graduate from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and function.
• Document in FOX's EMR system in line with regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
