Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist (PT) for part-time/per diem in-home visits. Join a trusted home health provider focused on keeping clients thriving at home.

You will perform in-home PT evaluations, craft personalized care plans, deliver interventions to improve strength and mobility, educate clients and families, collaborate with a multidisciplinary team, and document care using a mobile EMR.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle & license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
