FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in homes and senior living communities in Clear Lake, IA. You’ll have autonomy to schedule visits and tailor treatment plans to improve mobility and independence.

Responsibilities include evaluations, developing plans of care, and delivering evidence-based therapy, with opportunities to incorporate telehealth for broader access and continuity of care.

Formación

• Valid Physical Therapy license in the state(s) of practice.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilidades

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
