FOX Rehabilitation is expanding in Ames, IA, seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living settings. Clinicians will assess mobility and independence, create individualized plans, and provide evidence-based therapy within the FOX Geriatric House Calls™ model.

Qualifications

• Current Physical Therapy license in the state of practice or eligibility
• Basic computer and EMR proficiency
• Current CPR certification

Responsabilités

• Provide one-on-one physical therapy to older adults in private homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support mobility, independence, and functional goals
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
