GrandCare Health Services in West LA is seeking a Physical Therapist (PT) to deliver in-home care under a physician's plan of care, helping patients regain strength, mobility, and independence after surgery and other conditions.

You will perform home health PT evaluations, develop care plans, provide treatment, monitor progress, participate in discharge planning, supervise assistants as allowed, and enjoy flexible scheduling with mileage reimbursement and productivity bonuses.

Formación

• Current California Physical Therapy License is required.
• 2+ years of physical therapy experience.
• CPR certification required.
• Valid driver's license and insured vehicle required.

Responsabilidades

• Perform home health PT evaluations and develop care plans.
• Provide therapeutic treatment and instruct patients/caregivers.
• Monitor progress and report changes to physicians.
• Participate in discharge planning and interdisciplinary case conferences.
• Supervise Physical Therapy Assistants per state regulations.
