Job summary

GrandCare Health Services is seeking a part-time Physical Therapist to deliver in-home PT in West Los Angeles and surrounding areas. You will evaluate patients, develop care plans, and provide treatment in patients' homes, helping them regain mobility after surgery.

Requirements include a California PT license, CPR certification, and 2+ years of PT experience (home health preferred). Valid driver’s license and vehicle are required to travel between visits.

Qualifications

• Current California Physical Therapy License.
• 2+ years of physical therapy experience (home health preferred).
• CPR certification and valid driver’s license required.

Responsibilities

• Perform home health physical therapy evaluations and develop care plans.
• Provide therapeutic treatment and instruct patients/caregivers.
• Monitor progress and report changes to physicians.
• Participate in discharge planning and interdisciplinary case conferences.
• Supervise Physical Therapy Assistants per state regulations.
