Metro Physical & Aquatic Therapy seeks a motivated full-time Physical Therapist to provide in-home care across Suffolk County, NY. The role requires NY licensure and a willingness to travel within the designated area. Base salary ranges from $90,000 to $110,000 yearly, with comprehensive benefits.

Qualifications

• Graduation from an accredited Physical Therapist program.
• Licensed to practice Physical Therapy in New York state (or eligible).
• Ability to travel to patients' homes within the geographic area.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients and develop individualized in-home therapy plans.
• Collaborate with physicians, patients, and families on care goals.
• Prepare and implement customized therapy programs to restore function.
• Instruct family members and aides on therapy techniques.
• Provide timely, accurate documentation for each visit.
• Communicate discharge plans with patients and care teams.
• Deliver care in compliance with policies and professional standards.
