Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to provide in-home, skilled physical therapy across our service area. The role emphasizes personalized care, a flexible schedule, and strong collaboration with a supportive team.

Responsibilities include SOC/ROC/Recert/DC processes, patient goal-directed interventions, and education for patients and caregivers. Must hold California PT licensure and be willing to travel to clients’ homes.

Responsibilities

• Provide comprehensive, skilled physical therapy services per physician orders.
• Perform SOC, ROC, Recert, and DC evaluations and discharges.
• Assess patient needs, plan interventions, evaluate outcomes, and adjust care to goals.
• Collaborate with the interdisciplinary team to set and evaluate patient goals.
• Educate patients, families, and caregivers on PT programs and home exercise regimens.
