Qualifications

• 1+ year of PT experience in outpatient, rehabilitation, home health, or geriatric care.
• Current, unrestricted Florida physical therapist license.
• Ability to cover Osceola, Volusia, Seminole, and Orange/Brevard counties.

Responsibilities

• Travel to patients' homes to perform evaluations and assessments.
• Craft tailored physical therapy plans based on each patient's needs.
• Educate patients and caregivers on exercises and stretches.
• Provide assisted device support and ensure compliance with clinical documentation.
• Communicate patient progress and any changes to the physician and care team.
