The position offers above-market pay of $135,000 - $145,000 per year, flexible scheduling, and grouped visits to minimize drive time, with strong support from nursing and therapy teams.

Qualifications

• Current, unrestricted Florida physical therapist license.
• Ability to cover multiple counties (Osceola, Volusia, Seminole, Orange/Brevard).
• Open to candidates relocating from outside Florida who will obtain FL license.
• Preferred: 1 year of PT experience in outpatient, rehab, home health, or geriatric care.
• Experience performing home health evaluations and related documentation preferred.

Responsibilities

• Perform in-home physical therapy evaluations and create treatment plans.
• Improve mobility, strength, balance and functional independence; prevent falls.
• Walk patients and caregivers through exercises; educate on mobility techniques.
• Educate patients on safely using assistive devices; maintain documentation.

THE INS AND OUTS OF BECOMING OUR HOME HEALTH PHYSICAL THERAPIST

You'll travel to homes throughout our service area to assess patients' needs, develop effective treatment plans, and help deliver quality patient care. With a caring and compassionate approach, you'll perform in-home physical therapy evaluations and create custom plans focused on improving patients' mobility, strength, balance, and functional independence as well as preventing falls.

You'll walk patients and their caregivers through the exercises, teach them mobility techniques, and educate them on safely using assistive devices. As you meet with patients, you'll maintain accurate clinical documentation and promptly communicate their progress/condition changes to the care team and physician. Our Home Health Physical Therapist works closely with nursing, therapy, and administrative teams to maintain comprehensive care and ensure the timely completion of authorized visits.

Our Requirements

• Current, unrestricted Florida physical therapist license
• Ability to cover multiple counties (our service area includes Osceola, Volusia, Seminole, and Orange Brevard)
• We are also open to candidates currently outside of Florida who are planning to relocate and willing to obtain the FL PT license!

While not required, our preferred Home Health Physical Therapist has at least 1 year of physical therapy experience, ideally in an outpatient, rehabilitation, home health, or geriatric care environment. Experience performing home health evaluations and completing related documentation is also preferred.
