Our ideal In-Home Physical Therapist has at least 1 year of physical therapy experience in an outpatient, rehabilitation, home health, or geriatric care environment. Experience performing home health evaluations and maintaining accurate documentation would also be helpful. What matters most is that you have:
You'll work with our nursing, therapy, and administrative staff, helping empower patients to meet their physical therapy goals by showing them the correct steps to get there. As our In-Home Physical Therapist, you'll travel throughout our service area, perform evaluations and assessments in patients' homes, and use this information to craft tailored physical therapy plans. With a focus on mobility, strength, balance, and functional independence, you'll identify the most effective exercises for each patient's needs and educate the patient/caregivers on these stretches. You'll provide assisted device support, maintain compliant clinical documents, and communicate the patients' progress and condition changes to the physician and care team.
