Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Rahway, NJ, providing services in private homes or senior living settings. You will evaluate, plan, and execute evidence-based treatments while maintaining thorough documentation in FOX's EMR system.

The role offers autonomy in scheduling, collaboration with an interdisciplinary team, and opportunities to incorporate telehealth for flexibility and extended access to patients.

Qualifications

• Current Physical Therapy license in NJ or eligibility to obtain.
• Proficient with EMR systems and basic computer skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in private homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care to extend access when appropriate.
