Job summary

FOX Rehabilitation in South Lakewood, New Jersey seeks licensed Physical Therapists to deliver one-on-one geriatric care in homes or senior living communities. You’ll evaluate, treat, and document in FOX’s EMR, with autonomy to schedule and build patient relationships, including telehealth options to extend access.

Ideal candidates are team-oriented, value evidence-based practice, and want meaningful outcomes for older adults.

Qualifications

• Active Physical Therapy license in the state of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy to support wellness, mobility, and independence
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
